Earn $36.05/hr

The RN Consultant is responsible for treatment planning and providing case management services for the Nursing Home Diversion and Transition Case Management. The patient population covered will include individuals with mental health diagnoses, complex medical conditions, and/or social-economic and co-morbidities.

The ideal candidate will possess the following qualifications:

* Registered Nurse must possess a degree from an accredited school.
* This individual should have work experience in mental health systems and be comfortable working collaboratively as part of a team, in an integrated system.
* Valid Louisiana RN license-unrestricted
* Valid LA Driver’s License Appropriate state and federal clearances.
